When you call The Providence Center for an appointment, you will speak to an admissions specialist who will ask you:

  • Your name, address, telephone number and date of birth
  • The name of your insurance provider and your insurance ID number
  • To describe your current symptoms
  • What, if any, treatments you have had in the past
  • If you are taking medication

To ensure that you receive the most appropriate and best course of care, you will also be asked whether you have had any history of substance abuse.

If you are 17 years old or younger, your parent or legal guardian will need to call for an appointment for an initial assessment with our Child & Family Services division.

If you are 18 years old or older, you will receive an appointment with our Access Center.  Your initial assessment will be conducted by a psychiatric nurse or licensed clinician who will determine an appropriate placement for you. You will be given an appointment and provided with information about the program and services you will be receiving.
